Abstract

To determine the utility of fiberoptic bronchoscopy (FOB) and different FOB techniques for handling a specimen for early and late diagnosis in suspected active negative pulmanary tuberculosis (PTB) were aimed. In a prospective clinical study over a 19-month period, consecutive in- and outpatients of one University Hospital who were suspected to have active PTB were included. FOB with related subsidiary techniques when possible were applied. Between Tbc suspected and HIV negative 65 patients, out of 35 were found to have active PTB. FOB was diagnostic in 19/35 (54%). For early diagnosis, FOB was smear positive in eight (23%), pathology positive in two (6%), both smear and pathology positive in one (3%) and 11/35 (31%) in all patients. For late diagnosis pre-FOB sputum was culture positive in 2/35 (6%) and FOB was culture positive in 16/35 (46%). Application rates and microbiologic diagnostic performance of different techniques were for bronchial lavage 100-40%, bronchial biopsy 14-0%, brush 81-24%, BAL 14-20%, transbronchial biopsy 17-33%; postbronchoscopic sputum 64-43%. Granulomatous inflammation was shown in 3/12 (20%) TBB. Among techniques and evaluation methods no significant better tests were noted, rather they were complementary. FOB gives satisfactory yield of definitive and culture diagnosis of active PTB. FOB related techniques are complementary and must be performed as much of them whenever possible.
