Abstract
This is a case study of a 60 years old patient with mechanical jaundice ön tanı. In ultrassonography we founda a 20x25x25 mm, hyperchogenous, heterogenous mass like lesion in proximalis of the ductus hepaticus communis. Intrahepatic bilier duct were extensively ectazic. For a more detailed ex¬amination we performed percutan transhepatic cholangiography and saw "cut off sign" besides drifted sign of the periferical bilier ducts typical for clatskin tumour. Özet
Bu çalışmalarda mekanik ikter ön tanısıyla, hastanemize bas vuran 60 yaşındaki bir olguyla ilgilidir. Olguya ultrasonografi(USG) yapılmış ve duktus hepatikus kommunis proksimalinde yaklaşık 20x25x25 mm boyutlarında, hiperekojen, heterojen ekojenitede, düzensiz sınırlı kitlesel lezyon tespit edilmiştir. întrahepatik safra yolları ile derecede geniş bulunmuştur. İntraheptik safra yollarının daha iyi görüntülenebilmesi için perkutan transhepatik kolonjiografi(PTK) yapılmış ve duktus . hepatikus kommunis de "cutt-off sign" görünümünün yanında, klatskin tümörü için tipik olan periferik safra yollarının rüzgara kapılmış görünümü de izlenmiştir. Tetkikler esnasında total bülüribin düzeyi 33mg'a yükselen olguya, laparotomiye uygun olmaması nedeniyle eksternal bilier drenaj yapılmıştır.
