Abstract

Objective: To study the efficacy of fluoxetine and paroxetine on the level of depression-anxiety, quality of life, disability and metabolic control in patients with Diabetes Mellitus (DM) diag-nosed also as major depressive disorder. Method: The study was carried out with patients with a diagnosisof Diabetes Mellitus attending the Endocrinology Unit of İzmir Atatürk Training and Research Hospital consecutively and willing to participate. First the patients were screened with Hospital Anxiety-Depression Scale (HAD-A and HAD-D). By the psychiatric interview with patients with a total score above the cut-off point, patients diagnosed as major depressive disorder according to DSM-IV were subjected to Hamilton Rating Scale for Depression (HAM-D) and Hamilton Anxiety Rating Scale (HAM-A). Twenty five patients with a score of 13 or above on HAM-D were included to the study and evaluated with Short Form-36 (SF-36) and Brief Disability Questionnaire (BDQ), and their HbA1c levels were tested. A different psychiatrist started either a treatment of fluoxetine with a daily dose of 20 mg or paroxetine with a dose of 20 mg per day randomly. Patients were evaluated with the same scales in the 2nd, 4th, 6th and 12th weeks. HbA1c was re-tested at the end of the 12th week. Results: In both groups, statistically significant decrease in the HAM-D, HAM-A, GI, GI and BDQ scores was obtained, and quality of life was significantly improved. At the end of the treat-ment, while there was no statistically significant difference in the HbA1c levels, a slight decrease in the HbA1c level in the fluoxetine group was found. Conclusion : There is need for longitudinal studies with larger patient groups where other components of metabolic control of DM are evaluated. Thus, data on how depression in diabetic patients affects the prognosis of DM, what kind of relationship there is between depression and metabolic control, and whether some SSRI's have superiority to others in terms of improvement in metabolic control will be obtained.

Özet

Amaç: Major depresif bozukluk tanısı konmuş diabetes mellituslu (DM) hastalarda şuoksetin ve paroksetinin depresyon-anksiyete düzeylerine, yaşam kalitesine, yeti yitimine ve metabolik kontrol üzerine etkisinin araştırılmasıdır. Yöntem: Çalışma, DM tanısıyla İzmir Atatürk Eğitim ve Araştırma Hastanesi "Endokrinoloji Birimine"ne ardışık poliklinik başvurularından çalışmaya katılmayı kabul eden hastalarla yapıldı. Hastalara önce Hastane Anksiyete-Depresyon Ölçeği (HAD-A ve HAD-D) verildi. Eşik üstü puan alan hastalarla yapılan psikiyatrik görüşme sonunda DSM-IV ölçütlerine göre major depresif bozukluk saptananlara Hamilton Depresyon Derecelendirme Ölçeği (HAM-D), Hamilton Anksiyete Derecelendirme Ölçeği (HAM-A) uygulandı.
