Abstract

A 40-year-old multiparous female in her six th week of preg- nancy presented to our clinic with ex treme vaginal bleeding. On physical examination, the patient had cervical blood clots, normal uterine size, and no abdominal guarding or rebound. Adouble endometrial ring and irregular gestational sac on theleft cavity were detected via transvaginal ultrasound. The val- ues of β-hCG, progesterone, and hemoglobin were 40704 mIU/ mL, 15.5 ng/mL, and 9. 1 gr/dL, respectively. The left uterine cav- ity was unable to be reached; consequently, therapeutic curet- tage procedure was not successful. Endometrial sampling wastaken from only the right endometrial cavity and was sent forpathologic examination. Systemic methotrexate was adminis- tered weekly for regression of the cornual gestational sac. OnDay 28 of second dose methotrexate treatment, β-hCG levelwas decreased to 2.35 mIU/mL and gestational sac regressedspontaneously. The histopathologic evaluation was reportedas Arias-Stella reaction.

Beta-hCG 40704 mIU/mL, progesteron 15.5 ng/mL ve he- moglobin 9.1 gr/dL idi. Küretaj işleminde sadece sağ kaviteyegirilebildi, sol kaviteye girilemedi ve gebelik kesesi boşaltılamadı. Sağ endometrial kaviteden alınan örnek patolojik incelemeye gönderildi. Bikornuat uterusta kornual yerleşimli erken gebelik tanısı konulan vakaya iki doz haftalık metotreksattedavisi yapıldı. Sonrasında ayaktan takibe alınan hastanınβ-hCGsi ikinci doz metotreksat tedavisinin 28. gününde 2.35mIU/mle indi ve gebelik kesesinde spontan regresyon izlendi. Patoloji sonucu aries-stella reaksiyonu olarak rapor etti.
