Abstract

Objective: In this study, we reported the indications, results, and complications of genetic amniocentesis performed between 2000 and 2005. Also cost of a detected chromosomal abnormality by amniocentesis was calculated. Materials and Methods: We performed genetic amniocentesis during 2000-2005 period due to high risk in biochemical screening (³1/270), advanced maternal age ($geq$35 years of age), abnormal ultrasound findings and combination of more than one indication. Amniocentesis was performed between the 16-20 weeks of gestation under the ultrasound guidance. Retrospectively, data of a total of 843 cases were evaluated. Also we calculated the cost of detecting a chromosomal abnormality by multiplying the total cost of an amniocentesis with total number of amniocentesis and divided to total number of chromosomal abnormality which was detected by cytogenetic evaluation. Result: Median age was 34±5.9, median gestational weeks’ at amniocentesis was 18.4±2.7. We performed amniocentesis due to increased biochemical screening test risk (n=220, 26.1%), advanced maternal age (n=343, 40.4%), abnormal ultrasonographic findings (n=122, 14.4%), combination of advanced maternal age and positive biochemical screening test (n=132,15.5%), combination of high biochemical screening test and abnormal ultrasonographic findings (n=13, 1.5%), and other (n=15, 1.7%) reasons. We performed 49 amniocentesis to detect a chromosomal abnormality. Chromosomal abnormality was seen in 2.04% of the cases (n=17). Cultures were not succesfull in six cases (0.07%). We estimated that total cost to detect one chromosomal abnormality is 21 223 New Turkish Lira (YTL). Conclusion: Age-based performation of amniocentesis is not cost-effective. To decrease the higher false positive rate, combination of genetic ultrasound and biochemical screening seems to be the most cost effective alternative.
