Abstract

Churg-Strauss syndrome is a systemic, necrotizing vasculitis and it is the least common one among ANCA associatedvasculitis. The American Collage of Rheumatology classification criteria, patients having at least four of the following criteriaincluding asthma, eosinophilia (>10%), pulmonary infiltrates, mono-polyneuropathy, paranasal sinus abnormalities, extravascular eosinophils in biopsy specimens can be diagnosed as Churg-Strauss syndrome.Asthma, and migrating pulmonary infiltrates are seen inpulmonary involvement. Leukotriene receptor antagonistsmay also induce secondary vasculitis. Therefore, diagnosisof Churg-Strauss syndrome in patients with asthma whoare on treatment of such drugs is a challenge.Cresentic glomerulonephritis is seen rarely in cases withrenal involvement, and it is treated with immunosupressiveagents. In this case report, we presented a patient withChurg-Strauss syndrome developed concomitant to montelu -kast treatment for asthma, and presented as rapidly progressive glomerulonephritis which was treated successfully.

Özet

Churg-Strauss sendromu ANCA ilişkili vaskülitler arasındaen seyrek görülen, sistemik, nekrotizan bir vaskülit türüdür.Amerikan Romatoloji Cemiyetinin sınıflandırma kriterlerine göre astım, eozinofili (>%10), pulmoner infiltratlar,mono-polinöropati, paranazal sinüs anormallikleri, biyopside ekstravasküler eozinofiller görülmesi kriterlerden enaz dördünü karşılayan hastalara Churg-Strauss sendromutanısı konmaktadır.Akciğer tutulumunda klinik olarak astım, geçici pulmonerinfiltratlar gözlenir. Vaskülit lökotrien antagonistlerinesekonder gelişebileceği için astım tanısı ile mevcut tedaviyikullanan hastaların tanısında güçlük yaşanabilir.Böbrek tutulumunda nadiren kresentik glomerulonefritgelişir. Tedavisinde immunsupresif ajanlar kullanılır.Olgumuzda, montelukast kullanmaktayken gelişen, hızlıilerleyen glomerulonefrit şeklinde ortaya çıkan ve başarılıbir şekilde tedavi edilen Churg-Strauss sendromlu hastadan bahsedilmektedir.
