Abstract

The aim of this study was to evaluate the efficiency, adverse effects, cost of Iiposomai amphotericin B (LAMB, AmBisome®) therapy in febrile neutropenic episodes of children with cancer.. Between January 2003 and December 2004, in 17 of 96 neutropenic episodes LAMB was administered. In 23.5% of these 17 episodes, a fungal agent was identified either microbiologically and/or histopathologically. LAMB wass administered 1-5 mg/kg dose, mean 14 (7-46) days. Treatment success was 94.1% (16 episodes) and adverse effects were hypopotasemia (9 episodes), elevation of serum creatinin (2 episodes), asymptomatic hypocalcemia (1 episode) and acute allergic reaction (1 episode). All biochemical abnormalities were reversible and normalized in 5 days. Median therapy cost per episode was 21.710.148.000 TL (13740.6 USD) and median LAMB cost was 16.102.254.000 TL (10191.3 USD). In febrile neutropenic episodes, hospitilization period and LAMB administration were the main factors increasing cost, in conclusion, LAMB is a well-tolerated and safe agent in febrile neutropenic episodes of children with cancer. Reduction in the cost of manufacturing process will increase the avalibility of the drug.
