Abstract

Collection of fluid in pleural space can be in the form of pleural effusion, parapneumonic effusion, ampyema, transudate, exudate and complicated empyema. Pleural effusion secondary to pneumonia can cause emyema as a result of predisposing factors or inadequate treatment.63 cases with the diagnosis of parapneumonic effusion and empyema that have been followed up in our pediatrics clinic, between 1998-2003, are analyzed retrospectively with their laboratory and radiologic findings. Mean age of the patients was 6.01 years. 61.9 % of them were male and 38 % were female. 39 of the cases (61.9 %) had empyema and 24 of them (38.1 %) had pleural effusion. 17 of the cases (26.9 %) had history of antibiotherapy. In 9 of the cases S.pneumonia, in 3 cases Methisiline sensitive Staphylococcus Aureus, in 1 case Haemophilus influenza and in 1 case gram (+) coccus was detected in pleural fluid culture. 35 patients with empyema and 10 patients with pleural effusion were treated with chest tube drainage and parenteral antibiotics. 15 patients with pleuresia were treated only with parenteral antibiotics. Fibrinolytic therapy was performed in 2 patients with complicated empyema and 2 patients with empyema underwent decortication. The average duration of hospital stay was 30.3±9.9 days and clinical improvement was seen in 12.94±6.7 days.In conclusions; early diagnosis, appropriate antibiotherapy, early chest tube drainage together with effective antibiotherapy in cases with empyema decrease the morbidity and mortality. Fibrinolytic therapy and decortication must be considered if there is poor response to therapy.

Olguların yaş ortalaması 6.01 yıl olup, % 61.9'u erkek % 38.0'i kız cinsiyet oluşturmaktaydı. E/K = 39/24'dü. Olguların 39'unda ampiyem (% 61.9), 24'ünde plevral efüzyon (% 38.1) tespit edildi. Olgularımızın 17'sinde (% 26.9) daha önceden antibiyotik kullanım öyküsü mevcuttu. Ponksiyon sıvısında 9 olguda Streptococcus pneumoniae, 3 olguda metisiline duyarlı Staphylococcus aureus, 1 olguda Haemophylus influenzae, 1 olguda Gram (+) kok üredi. Otuzbeş ampiyemli ve 10 plörezili olguya tüp drenajı+parenteral antibiyotik, 15 plörezili olguya parenteral antibiyotik, iki komplike ampiyemli olguya fibrinolitik tedavi, ankiste ampiyem gelişen 2 olguya cerrahi tedavi (dekortikayon) uygulandı. Hastanede ortalama kalış süresi 30.3±9.9 gün klinik iyileşme süresi 12.9±6.7 gün olarak bulundu.Kliniğimizde parapnömonik olgulara yaklaşımda; erken tanı konulması, uygun antibiyoterapi, ponksiyon sıvısının analizi sonucu ampiyemi destekleyen olgularda etkin antibiyoterapi yanında erken tüp drenajı yapılması mortalite ve morbiditeyi azaltmaktadır. Etkin tedaviye rağmen yanıt alınamayan olgularda drenaj yetersizliği değerlendirilerek fibrinolitik tedavi ve dekortikasyon düşünülmelidir.
