Abstract
Recently, wide spreading use of dental implants in the prosthetic treatment of edentulous areas has increased the encountered possibility of peri-implant diseases. Peri-implantitis is an inflammatory process, including also bone loss in peri-implant tissues. Many local and systemic factors may play a role in its etiology. Patientrelated factors such as previously periodontitis history, diabetes, genetics, poor oral hygiene, smoking, alcohol consumption lack of keratinized soft tissue and the other factors such as selected implant system and type, surface properties of the implant, occlusal forces on implant and surrounding tissues, conditions encountered during the placement of implant are the possible risk factors for peri-implant diseases. Similar to periodontal diseases in the treatment of peri-implantitis, clinical and radiological diagnosis, non-surgical periodontal treatment, and also in addition to mechanical and chemical decontamination applications surgical periodontal treatment techniques such as flap operations, osteoplasty, ostectomy and guided bone regeneration has been usually applied. Günümüzde, dişsiz bölgelerin protetik tedavilerinde dental implantların kullanımının yaygınlaşması peri-implant hastalıklar ile karşılaşılma olasılığını da arttırmıştır. Peri-implantitis, implant çevre dokularında kemik kaybını da içeren inflamatuar bir süreçtir. Etiyolojisinde birçok lokal ve sistemik faktör rol oynayabilmektedir. Geçirilmiş periodontitis hikayesi, diabet, genetik özellikler, kötü oral hijyen, sigara kullanımı, alkol tüketimi, keratinize yumuşak dokunun yetersizliği gibi hasta ile ilişkili faktörler ve seçilen implant sistemi ve tipi, implantın yüzey özellikleri, implanta ve çevre dokulara gelen okluzal kuvvetler, implant yerleştirilmesi esnasında karşılaşılan durumlar gibi diğer faktörler peri-implant hastalıklar için olası risk faktörleridir. Peri-implantitis tedavisinde, periodontal hastalıklara benzer şekilde klinik ve radyolojik tanı, cerrahi olmayan periodontal tedavi ve ayrıca mekanik ve kimyasal dekontaminasyon işlemlerine ek olarak flep operasyonları, osteoplasti, osteoktomi ve yönlendirilmiş kemik rejenerasyonu gibi cerrahi periodontal tedavi teknikleri sıklıkla uygulanmaktadır. Bu olgu sunumunun amacı, geçirilmiş periodontitis hikâyesi olan bir erkek hastada dental implantın yerleştirilmesinden beş yıl sonra görülen peri-implantitisin klinik ve radyolojik teşhisi ile birlikte rejeneratif tedavi işlemlerini sunmaktır.
