Abstract

Purpose: In intensive care units, 10-20% of nosocomial bloodstream infections are composed of Candida species. Candida species of normal flora omits the skin barrier by the way of invasive catheter application or endothracheal intubation and cause infections. In this study, we aimed to determine the distribution and the antifungal susceptibilities of Candida species isolated from blood cultures. Material Methods: Amphotericin B, fluconasole and itraconazole susceptibilities and Minimum Inhibitory Concentrations (MIC values) were measured with M27-A2 broth microdilution method which is recommended by National Committee for Clinical Laboratory Standards (NCCLS).Results: Candida spp were isolated in blood cultures of 23 patients throughout one-year period. Six of them were children and 17 were adults. 16 patients (69%) were admitted at intensive care unit. The most prevalent agent was C. albicans (52%). Amphotericin MIC values were found pretty low. Fluconazole susceptibility was detected 96% in accordance with the other studies. Conclusion: Due to the highest mortality rate of candidemia, especially candida infections emerged in intensive care units should be followed in case of appropriate and sufficient antifungal treatment and the emergence of resistant Candida species

Özet

Amaç: Yoğun bakım ünitelerindeki tüm nozokomiyal kan dolaşımı infeksiyonlarının %10-20’si Candida türleri tarafından oluşturulmaktadır. Normal vücut florasında bulunan Candida türleri yoğun bakım hastalarında uygulanan invaziv kateterler, endotrakeal tüpler gibi girişimlerin uygulanmasıyla doğal bariyerleri geçerek infeksiyona neden olmaktadır. Çalışmamızda hastanemizde bir yıllık süre içersinde kan kültürlerinden izole edilen Candida türlerinin dağılımını ve antifungal duyarlılıklarını saptamayı amaçladık.Gereç ve Yöntemler: Candida türlerinin amfoterisin B, flukonazol ve itrakonazol’a duyarlılıkları National Committee for Clinical Laboratory Standards (NCCLS) tarafından önerilen M27-A2 buyyon mikrodilüsyon yöntemiyle yapılmış ve minimum inhibitör konsantrasyon (MİK) değerleri saptanmıştır. Bulgular: Bir yıllık süre içersinde toplam 23 hastanın kan kültüründen Candida izole edildi. Bu hastaların altısı çocuk, 17’si erişkindi. Hastaların 16’sı (%69) yoğun bakımda yatmaktaydı. En sık izole edilen etken C.albicans’dı (%52). Çalışmamızda Amfoterisin MİK değerleri oldukça düşük bulunmuş, diğer çalışmalarla benzer şekilde flukonazol duyarlılığı %96 olarak saptanmıştır. Sonuç: Kandidemilerde mortalite oranlarının yüksek olması nedeniyle, özellikle yoğun bakım ünitelerinde ortaya çıkan Candida infeksiyonları uygun ve yeterli antifungal tedavilerin yapılması ve antifungal ajanlara dirençli Candida türlerinin gelişmesi açısından takip edilmelidir
