Abstract

Ýntrathoracic goiters are mainly of the secondary type and primary intrathoracic goiters are rarely seen. Primary ectopic thyroid tissue is the result of abnormal migration of the thyroid gland. Ectopic thyroid tissue may be encountered anywhere from the foramen caecum to the lower neck. It is rarely seen in the mediastinum. We present a 60-year-old woman with primary ectopic posterior mediastial goiter and recurrent multinodular goiter after previous. She was euthyroid status. On exploration of the thyroid through old cervical incision, the right lobe was multinodular enlarged. Right lobectomy was performed. The ectopic mediastinal goiter which underwent excision via thoracotomy had no connection to the thyroid gland and the vascular supply from the left innominate vein and innominate artery. In conclusion, primary intrathoracic goiter should be considered in the differential diagnosis of mediastinal tumor. Ectopic intrathoracic goitre should be removed to rule out malignancy and treated compressive symptoms.

Mediastendeki kitlenin boyundaki tiroid dokusuyla ilişkisi yoktu. Torakotomi yapıldığında kitlenin kanlanmasını sol innominate arterden aldığı görüldü. Kitle total olarak eksize edildi. Sonuç olarak primer intratorasik guatr, mediastinal tümörlerin ayırıcı tanısında akılda tutulmalıdır. Ektopik intratorasik guatrlar bası semptomlarının giderilmesi ve malignite riski nedeniyle cerrahi olarak tedavi edilmelidir.
