Abstract

In the last few years urinary tract pathogens have been reported to become increasingly resistant to common used antibiotics, therefore we aimed to determine the prevalence of microorganisms and antibiotic resistance patterns of the gram negative microorganisms in our patients with urinary tract infection. Study samples were divided into two groups including outpatients and inpatients. Although, E. coli was the leading microorganism in both groups, its prevalence was 56.6 % in outpatients and diminished to 37.1 %in inpatients. In spite of decreasing rate of E coli among inpatients, microorganisms such as Klebsiella spp, Streptococcus spp, Enterococcus spp and Pseudomonas spp increased significantly in the former group. For Gram negative pathogens high resistance to ampisillin (69.3%), trimetoprim-sulfamethoxazole (46.1%) and relatively less resistance to amoxicillin- clavulanate (26.8%), cefuroxime (22.4%) was observed among outpatients. The resistance was between 54.0 - 79.7 % for these oral antibiotics in inpatients. In both groups, nitrofurantoin has been shown to have the lowest resistance rate, 1.1% in the outpatients and 11.1% in the inpatients. The resistance for amikacin was 9.5 % in the outpatients and 21.1% in the inpatients; ceftriaxone resistance was 16.4 % in outpatients and 48.8% in the inpatients. For inpatients overall resistance to most antibiotics was significantly higher than outpatients. Our results were similar to the literature and we may conclude that nitrofurantoin could be preferred in empirical oral treatment, whereas amikasin might be the choice parenteral treatment.
