Abstract

Aim: It is wellknow n that Magnetic Resonance Imaging (MRI) findings have themost determinant value in the evaluation of the risk for transition from clinicalisolated syndrome (CIS) to multiple sclerosis (MS). The objective ofthisstudywastoinvestigatethe effect and prognostic importance of MRI findingson the transitionto MS in baseline evaluation in the first clinical event.Material&Methods: Patients who were followed up with CIS at Dokuz EylulUniversity Neurology Department were included in the study. MRI examinationsperformed both in the attack period and once in eachsixmonthroutinely inpatients with CIS were evaluated in order to assess whet her McDonald 2010criteria were met in transmition to MS. Brain and spinal MRI examinations wereplanned as Flair Sagittal, Flair Axial, T2 axial, T1 axial and with Gd sequences.CIScases withnew/activelesion(s) found at follow-upMRIswereconsidered asdefinitive MS accordingto McDonald 2010 Criteria. Results:A total of 130 MS patients (92 female) who were followed up from thefirst clinic eventsanddefinitely diagnosed with MS accordingto McDonald orPosercriteria were evaluated. The meanage was 33.45±9.27 years. TransitiontoMS developed earlier in patients with black hole detected compared to thosewith out black hole detected. Similarly, patients with gadoliniumen hancementin the baseline MS were found toearlier transmitted to MS, compared to thepatients with out gadolinium enhancement. For the black hole, sensitivity wasfound as 55%, specificity as 62%, positive predictive value (PPV) as 51%,negative predictivevalue (65%), and accuracy as 59%. For the lesions withgadolinium enhancement, sensitivity was 98%, specificity 90%, PPV 88%, NPV98%, andaccuracy 93%.Conclusion: This study was important in terms of high values of PPV, NPV,sensitivity and specificity of the lesions with enhancement with out distinctionbetween symptomatic and a symptomaticcases, and emphasizing theimportance of McDonald 2017 criteria. Inaddition, low specificity of thepresence of corpus callosum lesion and black hole thatare clinically importantfort hediagnosis of MS mayexplained why these radiological parameters stilldidnot enter to McDonald criteria.
