Abstract

Alcohol withdrawal delirium (Delirium Tremens) is a complication, which could be seen %5 of patients hospitalized for alcohol dependence. It is a life threating condition.. It could appear in several hours after reduction or droping of alcohol. It mostly appears in the first 2-4 days of the treatment. The clinical symtomps are confusion, increase of psychomotor activation, autonomic signs (increase of blood pressure, sweating, palpitation, ect.), hallucinations, especially persecutary delusions, tremor and seizure.Alcohol withdrawal delirium has been observed in patients with high consumption at 40-50 ages. In pscyhiatric publications, several delirium tremens cases with elderly patients can be seen, however there are fewer studies with younger patients. The aim of this study is implying the contents of delirium tremens cases in younger ages. These cases consist of 5 male patients between 27- 29 years old, who are inpatient between 1996-2006 on Foundation of Balikli Rum Hospital, Anatolia Clinics Alcohol Treatment Center, with alcohol dependancy and alcohol withdrawal delirium diagnosis. The files of the patients are scanned retrospectively and sociodemographical data, alcohol use habits, laboratory rates, physical diseases, psychometric tests and comorbid psychiatric disaeses are observed. As a result, after alcohol intake immediate high consumption of alcohol and depression has been observed in younger people with delirium tremens diagnosis.

Özet

Alkol yoksunluğu deliryumu (Deliryum Tremens) hastaneye yatarak tedavi gören alkol bağımlılarında %5 oranında görülen ve yaşamı tehdit eden bir komplikasyondur. Kullanılan alkol miktarının azaltılması ya da kesilmesinin ardından birkaç saat içinde, çoğunlukla ilk 2-4 günde ortaya çıkar. Klinik tablo bilinç bulanıklığı, psikomotor aktivite artışı, otonomik aşırı uyarılmaya bağlı belirtiler (kan basıncında artış, terleme, çarpıntı vb.), değişik varsanılar, perseküsyon sanrıları, tremor ve epileptik nöbetlerle seyredebilir. Alkol yoksunluğu deliryumunun çoğunlukla 40-50 yaş aralığında, yoğun alkol tüketen hastalarda görüldüğü bildirilmektedir. Psikiyatrik yayınlarda çok sayıda ileri yaşlarda görülen deliryum tremens olgu örnekleri yer almasına karşın, görece genç yaşlarda görülen deliryum tremens hakkında çok az sayıda çalışma vardır. Amacımız, genç yaşlardaki deliryum tremens olgularının özelliklerini ortaya koyarak bu alanda bilgi birikimini artırmaktır.Bu olgu serisi Balıklı Rum Hastanesi Vakfı Anatolia Klinikleri Alkol Tedavi Birimi’nde 1999-2006 yılları arasında alkol bağımlılığı ve alkol yoksunluğu deliryumu tanıları ile yatırılarak tedavi edilen 27-29 yaşları arasındaki 5 erkek hastadan oluşmaktadır. Hastaların dosyaları geriye dönük olarak taranmış ve sosyodemografik özellikleri, alkol kullanma alışkanlıkları, laboratuar değerleri, fiziksel hastalıkları, psikometrik testleri ve alkol bağımlılığına eşlik eden diğer psikiyatrik hastalıkları incelenmiştir.Sonuç olarak genç yaşlarda görülen deliryum tremens olgularında alkole başladıktan kısa süre sonra yoğun miktarda alkol kullanımının oluştuğu (hızlı tolerans artışı) ve depresyon ek tanısının bağımlılığa eşlik ettiği görülmüştür.
