Abstract

Torsades de pointes (TdP), which is associated with aprolonged QT interval, is a rare but potentially fatalarrhythmia. The most common cause of drug-inducedQT prolongation is inhibition of the rapidly activatingcomponent of the delayed rectifier potassium current.Rapidly activating component of the delayed rectifierpotassium current inhibition delays repolarization byblocking the potassium in myocytes. In this article, wepresent a patient who had TdP risk factors including femalesex, organic cardiac disease, and bradycardia. On the fifthday of 400 mg/day oral moxifloxacin administration, the patient had an episode of TdP, which progressed toventricular fibrillation, and was successfully defibrillated.

Özet

Uzamış QT aralığı ile ilişkilendirilen Torsades depointes (TdP) nadir fakat ölümcül bir aritmidir.İlaçla indüklenmiş QT uzamasının en yaygın nedenigecikmiş düzeltici potasyum akımlarının hızlıkomponentinin inhibisyonudur. Gecikmiş düzelticipotasyum akımlarının hızlı komponentinin inhibisyonumiyositlerdeki potasyumu bloke ederek repolarizasyonugeciktirir. Bu yazıda; kadın cinsiyeti, organik kardiyakhastalık ve bradikardiyi içeren TdP risk faktörleri olanbir hasta sunuldu. 400 mg/gün oral moksifloksasinuygulamasının beşinci gününde hasta ventrikülerfibrilasyona ilerleyen bir TdP epizodu geçirdi vebaşarıyla defibrile edildi.
