Abstract

Background/aims: Duodenal ulcer recurrence is a frequently encountered problem in the disease. Ulcer recurrence is related to factors such as quality of mucosal healing, degree of mucosal inflammation and scar formation in the healing tissue. Helicobacter pylori infection is reported to have a role in poor ulcer healing. Methods: The aim of this study was to evaluate the differences in ulcer healing rates and the quality of histopathologic healing and to investigate the effects of Helicobacter pylori eradication on the quality of ulcer healing, in two groups of Helicobacter pylori (+) duodenal ulcer patients. Group I (omeprazole only group) comprısed 23 patients who were prescrıbed omeprazole 20 mg daily for four weeks and Group 2 (Helicobacter pylori eradication group) comprised 23 patients prescribed omeprazole 20 mg daily for four weeks, clarıthromycin 500 mg bid for ten days and amoxycillin 1 gr bid for 10 days. In these patients, the quality of the ulcer scar formed was evaluated using a semi-quantitative scale with the following parameters: histologic maturity of epithelial cells, pattern of connective tissue, severity of inflammation, activity of inflammation. Results: There were no differences in the rates of ulcer healing and scar tissue formation in either group. The Helicobacter pylori eradication rate was significantly higher in group 2 (23/23) than in group 1 (3/23) (p<0.05). The total scores of ulcer scar tissue were lower in group 2 (126 points) than in group 1 (178 points) (p<0.05). In addition, each score for every parameter of the semi-quantitative scale for ulcer scar tissue was lower in group 2 than in group 1 (p<0.05). There were differences (but not statistically significant) between endoscopic and histopathologic healing rates in both groups. six of 15 patients with complete endosco-pic healing in group 1 and five of 18 patients with complete endoscopic healing in group 2 was found to have poor histopathological healing. Conclusions: Our findings reveal that Helicobacter pylori infection is one of the causes of poor histopathologic healing in duodenal ulcer disease and that the quality of ulcer healing is better in patients given Helicobacter pylori eradication treatment.
