Abstract

Background/Aims: Although hepatobiliary cysts have been reported in patients with autosomal dominant polycystic kidney disease, the possible relationship between biliary tract abnormalities and this disease has not been formally investigated. The aim of this study was to assess the incidence of biliary tract abnormalities in autosomal dominant polycystic kidney disease patients. Methods: Twenty six autosomal dominant polycystic kidney disease and 26 chronic renal failure patients matched for age, gender and duration of hemodialysis were included in the study. All subjects' biliary tracts were evaluated using magnetic resonance cholangiopancreatography. In cases where magnetic resonance cholangiopancreatography revealed biliary problems, patients who consented to further investigation were also assessed with endoscopic retrograde cholangiopancreatography. Results: Bile duct dilatation, defined as visible intra- hepatic bile ducts and/or extrahepatic bile ducts greater than seven mm in diameter, was observed more often in autosomal dominant polycystic kidney disease patients (15/26; 57.6%) than controls (4/26; 15.4%) (p=0.002). Nine of the 15 autosomal dominant polycystic kidney disease patients with bile duct abnormalities were evaluated by endoscopic retrograde cholangiopancreatography. Sludge and a stone were extracted from two and one of these eight patients respectively. Three others exhibited intrahepatic biliary dilatation while three patients had normal endoscopic retrograde cholangiopancreatography findings. Conclusions: The study demonstrates that there is a high incidence of bile duct abnormalities in autosomal dominant polycystic kidney disease patients. Since both chronic renal failure syndrome and renal transplantation weaken the immune system, it is important to keep in mind that the biliary tract is a potential source of infection in individuals with autosomal dominant polycystic kidney disease.
