Abstract

Omeprazole (OMP) suppresses gastric acid secretion by blocking H+-K+ ATPase in the parietal cell. It has been reported that oral OMP, but not intraperitoneal (ip) OMP (both forms in antisecretory doses), prevent acute ethanol-induced gastric mucosal lesions in the rat. Pentoxifylline (PTX), a methylxanthine derivate, improves red cell deformability, decreases platelet and red cell aggregation, inhibites neutrophil adhesion to cultured endothelium cells and improves microcirculatory blood flow. The aim of this study was to investigate the effects of PTX, OMP and PTX+OMP on ethanol-induced gastric mucosal lesions. Methods: The rats were divided into four groups: Group 1 (OMP group, n=9 rats) was pretreated with OMP 30mg/kg ip. Group 2 (PTX group, n=9 rats) was pretreated PTX 100mg/kg ip, Group 3 (PTX+OMP group, n=10 rats) was pretreated with both drugs at the same dosage and Group 4 (control group, n=9 rats) was pretreated with saline in 1ml ip doses. After 90 minutes, 2 ml of ethanol 98% was administered intragastrically to all groups. Sixty minutes later, all rats were killed and examined for lesion lengths, gastric juice pH and histopathologic evaluation. Results: A. pH values sequentially from group 1 to 4 were 7.88±0.33, 3.08±0.62, 7.61±0.24 and 6.05 ±0.35 respectively. There were significant differences between the control and other groups (p<0.002), between group 1 and 2 (p<0.002) and between group 2 and 3 (p<0.0002). B. Lesion lengths were 49.5±9.9, 19.7±4.5, 24.9±0.6 and 57.8±9.2 mm. The lesion lengths of the PTX and PTX+OMP groups were different from the control and OMP groups and that difference was significant (p<0.002). C. The values obtained by scoring from histopathologic evaluation were as follows: 1.44±0.29, 1.61 ±0.34, 1.40±0.22 and 1.55±0.33 respectively and there was no significant difference between the groups (p>0.05). In conclusion, PTX and OMP+PTX protect against ethanol induced gastric injury rather than OMP alone but there is no statistical difference in microscopic score between the groups.
