Abstract

In spite of great advances in the etiology and managament of duodenal ulcer, it remains an important health problem. The annual incidence of bleeding, which is the most common and important complication in duodenal ulcer, ranges between 0.006 % and 0.009%. We studied 19 patients with duodenal ulcer disease and nine control cases attending the Gastroenterology outpatient clinic at Türkiye Yüksek İhtisas Hospital. Upper gastro-duodenoscopic investigation of all control patients was normal. Fibrinolytic parameters were evaluated in the peripheric venous blood and the duodenal bulb tissue of both the patient and control groups. Fibrinolytic parameters in venous blood were studied for plasminogen (Plg), alfa-2 antiplasmin (alfa 2AP), tissue plasminogen activator (t-PA), plasminogen activator inhibitor-1 (PAI-1) and fibrinolytic activity (FA) in each group. Tissue fibrinolytic parameters were also investigated in the patient and control groups. Biopsies were taken from the margin and peripheric hyperemic region of the duodenal ulcer in patients and from normal duodenal bulb mucosa in control cases. FA and t-PA were studied in all samples. When the parameters of fibrinolytic activity in the blood were compared in each group , no significant difference was found. However, t-PA and FA levels in the active doudenal ulcer patients were higher than those of control cases. Margin levels in particular were more increased (margin level of D.U, t-PA p< 0,00005, hyperemic region t-PA, p<0,05). FA was found to be significantly higher at the edge of ulcer).
