Abstract

The prevalence of patent foramen ovale (PFO) has been shown to be higher in patients with ischemic stroke than in control subjects. Paradoxical embolization is thought to be a potential mechanism f or ischemic stroke in patients with PFO. Between January 1995 and July 1998, we found PFO in four patients (3F, 1M) with ischemic stroke using contrast transesophageal echocardiography (TEE). The age of patients ranged from 29 to 60 years. Two patients had an infarct consistent with single arterial territory while two patients exhibited infarcts of multiple arterial territories. One patient had a history of recurrent ischemic strokes. All patients' transthoracic echocardiographic (TTE) examinations were normal and no other etiologic factor was found for ischemic stroke. TEE examinations revealed the following findings: PFO and hypermobility of atrial septum (n=1); PFO and mitral valve prolapse (n=1), PFO and Ebsteinis anomaly (n=1); and PFO, calcification in mitral ring and sclerotic changes on aort valve (n=1). As a conclusion, we suggest that performing the TEE in ischemic stroke patients in whom no risk factor could be found, may reveal further information to find out the etiology.
