Abstract

Objective: It was aimed to treat proximal tubal obstructions of infertile patients with hysteroscopic transcervical tubal cannulation. Institution: SSK Aegean Maternity and Womens' Health Teaching Hospital, II. Gynecology Clinic, Izmir Materials and Methods: Twentyone patients, 20-40 years old, who were admitted to our infertility clinic between April 1999 and August 2001 and had cornual or proximal tubal occlusions on hysterosalpingography (HSG) and laparoscopy were included in the study. Laparoscopy and hysteroscopy were performed 40-50 days after subcutaneous injection of a single dose of Gonadothropin Releasing Hormone analogue (10,8 mg.). Tubal cannulation was performed with 3 Fr. ureteral catheter. Tubal patency was shown laparoscopically by methylene blue passage through the catheter. Patency was evaluated with HSG's taken 3 months postoperatively. Patients were followed up for 14,46 months (9-20 months}for pregnancy. Results: In 17 patients (80,95%) the ostiums were visualized. In three patients due to dense intrauterine adhesions and in one due to fundal 3mm. perforation occuring during cervical dilation, the ostiums were not observed. Catheterization was performed successfully in 13 patients unilaterally and in two bilaterally. Success rate was calculated as 88.24% . No peroperative tubal perforation was recorded. The tubal patency rate after 3 months postoperatively was 80.00%. After 15 successful tubal catheterizations, five pregnancies were recorded. The tubal patency was also accomplished in the second hysteroscopy of the patient with uterine perforation and pregnancy was reported. The total pregnancy rate was 37.50% . Conclusion: Hysteroscopic transcervical tubal cannulation must be performed prior to microsurgical anastomosis or In Vitro Fertilization in the treatment of tubal obstructions since it is a diagnostic, minimally invasive, intra-operative choice with therapeutic potential providing high tubal patency rates with low complication rate.

Hastalar ortalama 14 - 46 ay (9-20 ay) gebelik açısından takip edildi. Bulgular: Kanülasyon planlanan 21 hastanın İTsinde (%80,95) ostiumlar; 12 olguda bilateral, 5 olguda unilate¬ral olarak gözlendi. Üç hastada yoğun intrauterin adezyonlara bağlı olarak, bir hastada internal servikal ostaki rijid darlık aşılmaya çalışılırken meydana gelen fundal yüzdeki 3mm'lik perforasyon nedeni ile ostiumlar vizualize edilemedi. Onüç hastaya unilateral, iki olguya bilateral tubal kanülasyon başarıyla uygulandı. Toplam başarı oranı %88,24 olarak kaydedildi. Peroperatif tubal perforasyon izlenmedi. Postoperatif 3. aydaki tubal patens oranımız %80,00'dır. Onbeş başarılı tubal kateterizasyon sonrası, ortalama 14,46 aylık izlemde 5 gebelik kaydedilmiştir. Bununla beraber perforasyon meydana gelen hastamızda ikinci seansta tubal patens sağlanmış ve gebelik elde edilmiştir. Toplam gebelik oranımız %37,50 olarak hesaplanmıştır. Sonuç: Histeroskopik transservikal tubal kanülasyon;diagnostik, minimal invaziv, terapötik potansiyelli intraoperatif bir seçenek olması ve yüksek tubal patens oranlarını düşük komplikasyon oranı ile sağlaması açısından, tubal hastalıkların tanı, ayırıcı tanı ve tedavisinde mikrocerrahi anastomozdan veya In Vitro Fertilizasyondan önce denenmelidir.
