Abstract

Objective: To investigate the severity of pain and diurnal changes in uterine artery blood flow in cases with primary dysmenorrhea. Material and method: The study included 17 cases diagnosed as primary dysmenorrhea and 15 individuals who had no dysmenorrhea complaint. Bilateral Systole/Diastole rates (S/D), Pulsatility Index (Pl) and Resistance Index (Rl) measurements of all cases were made using transvaginal color Doppler at 12.00-02.00 p.m. and 12.00-02.00 a.m. Pain severity was evaluated at the same hours in dysmenorrhea cases using a verbal pain assessment scale. Wilcoxon Rank test was used in the statistical evaluation. P<0.05 was considered significant. Findings: Doppler measurements in dysmenorrhea cases at 12.00-02.00 p.m. showed that right uterine artery S/D: 3.27 ± 0.24, Rl: 0.74 ± 0.06, Pl: 2.41 ± 0.24: left uterine artery S/D: 3.32 ± 0.27, Rl: 0.72 ± 0.13, PI: 2.41 ± 0.24, whereas the measurements carried out at 12.00-02.00 a.m. revealed that right uterine artery S/D: 3.9 ± 0.14, Rl: 0.91 ± 0.16, Pl: 2.92 ± 0.21: left uterine artery S/D: 3.92 ± 0.27, Rl: 0.94 ± 0.17, PI: 2,94 ± 0.21. The comparisons demonstrated that all Doppler indices significantly increased in dysmenorrhea cases (p<0.05). There was no such significant increase in cases without dysmenorrhea (p>0.05). Pain score obtained from the verbal pain assessment of dysmenorrhea cases was 3.6±L4 during the day and 5.8± 1.7 at night. The difference was significant (p<0.05). Conclusion: Uterine artery blood flow decreases at night in women with dysmenorrhea. Therefore, less pain is felt at night. Our findings can provide a new perspective in arranging the working hours of women with dysmenorrhea and increasing productivity.

Der. 2005; 13: 345-348)
