Abstract

In this study, we investigated to determine the incidence of perioperative myocardial ischemia and ventricular repolarisation abnormalities in cataract surgery patients with known ischemic cardiac disease or have multiple risk factors for ischemic heart diseases, who are under sedation with remifentanil or propofol and compared them with placebo. Remifentanil infusion was started to Group Remifentanil (0.03-0.04 µg kg min-1) and propofol infusion to Group Propofol (1-2 mg kg h-1) 10 minutes before retrobulbar block. No medications were applied to Group Control. ECG with 12 derivations, electrolytes, and cardiac enzymes (troponin T and I) for all patients were obtained preoperatively, at the end of the operation and at the 12th hour postoperatively. Both systolic and mean blood pressures decreased significantly within time but only systolic blood pressures at the 10th, 20th, and 40th minutes and mean blood pressures at the 30th minute were different between remifentanil and propofol groups (P<0.05). ECG changes or cardiac enzyme increases which could favor ischemia were not detected. There were no significant differences between the pre-discharge QT maximum, QT minimum and QT dispersion values between propofol and remifentanil groups. Postoperative QT maximum and QT minimum values in propofol and remifentanil groups were found to be significantly longer when compared to control group. Remifentanil and propofol could be used safely for sedation in old patients who carry risk factors for ischemic heart disease.
