Abstract

Objectives: Mitral valve prolapse (MVP) displays a clinical picture closely resembling to panic disorder (PD) because of its symptoms like chest pain, tachycardia, arythmia, shortness of breath, dizziness, fatigue and anxiety. These similarities have lead to a consideration that there might be a causal relation between these two disorders. The purpose of this study was to evaluate the association between MVP and PD from multiple aspects and to illuminate the nature of the association. Method: 50 MVP patients, 50 PD patients and 50 healthy controls were included in this study. Echocardiographic evaluation were done to all subjects to diagnose MVP, and if present the level of prolapse. SCID-Clinical Version (CV) were administered to all subjects for clinical diagnosis, besides clinical symptoms of panic attack were specifically assessed. For psychometric evaluation, all cases completed Symptom Check List (SCL-90-R), Beck Depression Inventory (BDI), State and Trait Anxiety Inventory (STAI-1 and STAI-2). Results: MVP and PD cases were within similar age range like female/male ratio which was 2/1. PD prevalence was 16% in MVP cases, 2% in healthy controls with statistically significant difference in between. In all psychometric scales administered, average scores were highest in PD group whereas average scores in MVP group were higher than healthy control group. A/I differences were significant. MVP and PD cases display similar features pertaining to panic attack symptoms. MVP prevalence among PD cases was 12%. Conclusion: The present study has shown that the clinical symptoms and the age range observed in MVP cases were quite similar to PD cases. The prevalence of PD among MVPs was higher than normal population, and both MVP and PD groups had more psychopathology than healthy control groups. There was no relationship between the degree of prolapse and psychopathology and clinical symptoms. An overall assessment of these results would reveal that MVP and PD association should be treated as an important relationship that should be taken into account instead of a specific and unique association.
