Abstract

Objectives: We evaluated our surgical approaches to paranasal sinus tumors extending to the anterior skull base. Patients and Methods: The study included 110 patients (72 males, 38 females; mean age 51 years) who underwent surgery for paranasal sinus tumors with extension to the anterior skull base. The patients' ages ranged from three months to 76 years. Results: The tumors were benign in 67 patients, and malignant in 43 patients. Surgery included transfacial approaches (n=98), infratemporal approach (n=2), and a combination of transfacial and transcranial approaches (n=10). Postoperative complications included palate fistula in one case, facial paralysis in one case, longterm cerebrospinal fluid leakage in one case, and crusting of the nasal and sinus mucosa in patients in whom midfacial degloving approach was employed. One patient who underwent total maxillectomy died due to bleeding in the first postoperative month, another died in the intensive care unit on the first postoperative day. Non-Hodgkin's lymphoma developed in one patient who received postoperative radiotherapy. Conclusion: Skull base surgery has undergone considerable improvement due to the utilization of technologic advances, new reconstruction materials, and surgical techniques.
