Abstract

Objective: To compare the effectiveness of the new extrusion set and the removable valved trocar method and the routinely used silicone oil extrusion method by the vitrectomy machine for the high viscosity silicone oil removal. Material and Method: Twenty-five eyes of 25 patients who underwent silicone oil removal by extrusion set and removable valve trocar (Group 1) and 25 eyes of 25 patients who underwent silicone oil removal by silicon extraction cannula from a valved trocar using a vitrectomy machine (Group 2) was included in the study. The silicone removal process time of these patients, who have applied 5000 centistokes silicone in their first surgery, was calculated when the silicone removal process started. Complications developed during the procedure were noted. Results: All patients were operated using a 25 gauge vitrectomy system. The mean age of the patients in group 1 was 54.52±11.23 (35-75) years, while the mean age of the patients in group 2 was 52.44±11.93 (29-72) years (p=0.963). Silicone oil tamponade duration was 4.52±1.09 months in the first group, while it was 4.40±1.12 months in the second group (p=0.791). In the first group, silicone oil removal time was 4.59±0.46 (4.00-5.53) minutes, while it was 7.32±0.68 (6.53-8.47) minutes in the second group, and a significant difference was determined between the groups (p=0.027). While 3 (12%) of the patients in the first group had retinal re-detachment, 2 (8%) of the patients in the second group had retinal re-detachment after silicone oil tamponade removal. Conclusion: Silicone removal procedure with the extrusion set and removable valve trocar, which is performed without narrowing the trocar lumen, is an effective, reliable, and practical method. It enables high viscosity silicone oil removal from the small diameter trocar in a short time. New developments in vitreoretinal surgical techniques contribute to surgical practice.

Özet

Amaç: Yüksek viskoziteli silikon yağının çıkarılmasında ekstrüzyon seti ve çıkarılabilir valfli trokar kullanılan yeni yöntem ile rutin olarak uygulanan vitrektomi cihazı kullanımıyla silikon alma yönteminin etkinliklerinin karşılaştırılması. Gereç ve Yöntem: Silikon yağı aldırmak için başvuran hastalardan, silikon yağı alımı esnasında ekstrüzyon seti ve çıkarılabilir valfli trokar kullanılarak alım işlemi yapılan 25 hastanın 25 gözü ile (Grup 1) rutin olarak silikon alma/verme kanülünün kullanılarak valfli trokardan alım işlemi yapılan 25 hastanın 25 gözü (Grup 2) çalışmaya dahil edildi. İlk ameliyatlarında 5000 centistok silikon uygulanan bu hastaların, silikon alma işlemi başladığı anda silikon alma işlem süresi hesaplandı. İşlem esnasında gelişen komplikasyonlar not edildi.
