Abstract

Objective: To compare the anatomical and functional results of intravitreal afliberceptin, ranibizumab, and bevacizumab after loading dose in newly diagnosed diabetic macular edema cases. Material and Method: 90 patients with newly diagnosed diabetic macular edema were included in the study. Group1 consisted of 30 patients treated with aflibercept, group 2 consisted of 30 patients treated with ranibizumab, and group 3 consisted of 30 patients treated with bevacizumab. Three doses of intravitreal injections were applied to the patients at one month intervals. The best corrected visual acuity, central macular thickness, ganglion cell layer, internal nuclear layer, retinal pigment epithelial layer, outer nuclear layer before and after each injection ) values were compared. Results: While best corrected visual acuity increased statistically significantly in all three groups after 3 months of injection, there was no difference in efficacy when the increase in vision was compared between the three groups (p<0.001 and p=0.696). The initial central macular thickness values in the groups were 440.00 ± 128.81 µm, 495.83±130.42 µm and 486.33±166.71 µm, respectively, while the final loading dose was 354.03±168.08 µm, 343.22 ± 141.39 µm 373.46±185.47 µm, respectively. On the other hand, central macular thickness values significantly decreased in all three groups after injection compared to baseline values (p<0.001). When we examined the change in macular layers, no significant change was observed in ganglion cell layer, internal nuclear layer and retinal pigment epithelial values after injection, while outer nuclear laye values were significantly decreased in all three groups after injection (p=0.265, p= 0.205, p=0.093 and p=0.01, respectively). Conclusion: Anatomical and functional efficacy of bevacizumab, ranibizumab and aflibercept were found to be similar after the loading dose in the treatment of diabetic macular edema.
