Abstract

Aim: The effects of intravenous aminoacid infusion on surgical stress response, thermoregulation and gastrointestinal functions in abdominal aortic surgery in patients during general or combined general/epidural anaesthesia are investigated. Method: Forty patients were randomly divided into four groups as; general anesthesia with or without aminoacid infusion (Group 1 and 2, n=10), combined general/epidural with or without aminoacid infusion (Group 3 and 4, -1 n=10). Intravenous aminoacid -1solution of 80 g.L was infused at 2.5 mL.kg-1 .h for a total of 8 h commencing 2 h prior to the surgery. Klinik Çalışma General anesthesia included intravenous remifentanil, rocuronium and sevoflurane. The lumbar epidural was introduced prior to induction. After 10 mL of 0.25 % bupivacaine bolus dose, an infusion of 0.25 % bupivacaine at 4 mL.h-1 was continued for 24 hours. Heart rate and arterial blood pressures were recorded intraoperatively every 10 minutes, 1 and 24 hour postoperatively. The surgical stress response was evaluated by recording plasma glucose and cortisol levels preoperatively, 1 and 24 h postoperatively. Time to first flatus and defecation were recorded. Results: Five and fifteen minutes after intubation, the mean arterial blood pressures were higher in group 3 and 4, in comparison to group 1 and 2 however, mean heart rate was lower at five minutes after intubation in group 3 and 4, in comparison to group 1 and 2 (p<0.05). Comparing plasma glucose and cortisol levels, there were no significant difference between groups. No differences were observed regarding gastrointestinal functions, temperature changes and complications (p>0.05). Discussion: Amino acid infusion during surgery did not have significant effects on surgical stress response, gastrointestinal or thermoregulatory functions during abdominal aortic surgeries.
