Abstract

Lipid embolism occurs with the free lipid molecules which are derived from soft tissue and bone marrow during serious direct, surgical or soft tissue trauma. A morbid obese and diabetic patient having lumbar disc hernia was operated. L 5 -S 1 discectomy + Posterior L 5 osteotomy + Bilateral L 4 laminectomy + stabilization operation wa s performed. During stabilization with titanium plate, the patient had cardiac arrest. Although resuscitation was done, the patient died. Autopsy was done to learn the cause of death. At the autopsy, th e only positive sign was the diffuse lipid embolism of the lung. In conclusion, care should be taken to avoid lipid embolism during the spinal operations of diabetic and obese patients. If the patient dies, an autopsy must be done to determin e the cause of death.

Özet

Ya ğ embolisi, ciddi travma ve/veya cerrahi i ş lemlerden sonra kemik ili ğ inden ya da yumu ş ak dokulardan lipid globüllerinin sal ı nmas ı ile olu ş maktad ı r. Bu olgu sunumunda; diabeti mevcut, morbid obez, bel a ğ r ı lar ı olan kad ı n hastaya yap ı lan medikal tedavilere ra ğ men a ğ r ı lar ı nda düzelme olmay ı nca bel f ı t ı ğ ı ameliyat ı uygulanmas ı na karar verildi. Ameliyatta, sol L 5 -S 1 diskektomi, L 5 posterior osteotomi, L 4 bilateral laminektomi ve stabilizas yon operasyonu uyguland ı . Titanyum plak ile stabilizasyon esnas ı nda kardiak arrest geli ş mesi üzerine yap ı lan resüsitasyona yan ı t al ı namayarak hasta öldü. Ölüm sebebinin belirlenmesi için otopsi yap ı ld ı . Postmortem histopatolojik incelemede, akci ğ erlerde yayg ı n ya ğ embolisi bulgular ı d ı ş ı nda ba ş ka ölümcül bulgu saptanmad ı . Sonuç olarak, morbid obez ve diabetli hastalarda spinal ameliyatlar esnas ı nda ya ğ embolisi olma riski göz önünde bulundurulmal ı ve ölen olgularda ya ğ embolisi de dü ş ünülerek mutlaka ölüm sebebinin tespiti aç ı s ı ndan otopsi yap ı lmal ı d ı r.
