Abstract

Aim: The aim of this study was to determine the distribution of Candida species isolated from clinical samples and their susceptibility to various antifungal agents. Materials and Methods: A total of 138 Candida samples isolated between 2008 and 2010 from various clinical specimens were included in the study. The API ID 32 C identification system and ATB FUNGUS 3 kits were utilized for identifying Candida species and determination of their susceptibility to various antifungal agents. Results: Isolated Candida strains were identified as Candida albicans 92 (67%), Candida parapsilosis 24 (17%), Candida glabrata 10 (7%), Candida tropicalis 6 (5%) and other species (Candida kefyr, Candida lusitaniae, Candida colliculosa, Candida pulchherrima) 6 (4%). Whereas resistance rates of Candida albicans species were found to be 80% to itraconazole, 66% to voriconazole, 59% to fluconazole and 4% to amphotericin B, resistance to flucytosine was not detected. Although resistance rate of Candida parapsilosis strains to amphotericin B was determined as 17%, they were susceptible to all other tested antifungals. While Candida glabrata strains were fully susceptible to flucytosine, their resistance rates to other antifungals were found to be 10% to voriconazole, 20% to amphotericin B, 20% to fluconazole and 100% to itraconazole. Whereas C.tropicalis strains were fully resistant to fluconazole, itraconazole, voriconazole and fully sensitive to amphotericin B, their resistance rate to flucytosine was determined as 17%. Conclusions: Resistance rates to antifungal agents of Candida species isolated from our area were found considerably different compared to the other regions. Consequently, in patients suspected of having Candida infection, species identification and antifungal susceptibility testing must be performed and according to the results, treatment protocol must be corrected.

Özet

Amaç: Çalışmada; klinik örneklerden izole edilen kandidaların tür dağılımı ve çeşitli antifungal ajanlara karşı duyarlılığının belirlenmesi amaçlandı. Gereç ve Yöntem: 2008-2010 tarihleri arasında çeşitli klinik örneklerden izole edilen 138 kandida türü çalışmaya dahil edildi. Kandida suşlarının tür tayini ile çeşitli antifungal ajanlara duyarlılığının araştırılmasında API ID 32 C identifikasyon sistemi ve ATB FUNGUS 3 kitlerinden yararlanıldı. Bulgular: İzole edilen kandidaların 92 (%67)'si Candida albicans, 24 (%17)’ü Candida parapsilosis, 10 (%7)’u Candida glabrata, 6 (%5)’sı Candida tropicalis ve 6 (%4)’sı diğerleri (Candida kefyr, Candida lusitaniae, Candida colliculosa, Candida pulchherrima) idi. Candida albicans türlerinde itrakonazole %80, vorikonazole %66, flukonazole %59 ve amfoterisin B’ye %4 oranında direnç saptanırken, flusitozine karşı direnç saptanmadı. Candida parapsilosis suşlarının Amfoterisin B’ye karşı direnç oranı %17 olarak belirlenirken, suşların tamamı diğer antifungallere duyarlı bulundu. Tüm suşların flusitozine duyarlı olduğu Candida glabrata’nın, diğer antifungallere karşı direnç oranları sırasıyla; vorikonazol %10, amfoterisin B %20, flukonazol %20 ve itrakonazol %100 olarak bulundu. Candida tropicalis suşlarının tamamı flukonazol, itrakonazol ve vorikonazole dirençli, amfoterisin B’ye duyarlıyken, flusitozin direnç oranı %17 olarak saptandı.Sonuç: Bölgemizde izole edilen kandida türlerinin antifungal ajanlara duyarlılık oranlarında diğer bölgelere oranla büyük farklılıklar saptanması, kandida enfeksiyonlarından şüphelenilen durumlarda tür tayini ile antifungal duyarlılık testlerinin yapılmasını ve tedavi protokolünün buna göre düzenlemesini zorunlu kılmaktadır.
