Abstract

Missed vascular injuries following blunt traumas can lead to fatal clinical conditions that require an emergency intervention. Aortic transection (AT) is the most fatal complication of these missed vascular injuries. In this case report an AT that developed following a blunt trauma is presented. The patient was admitted with effort dyspnea and tachycardia. He had a history of blunt trauma three years ago. There was an enlargement of the upper mediastinum on X-ray studies. Thoracal magnetic resonance imaging and computed tomography revealed aneursym of the descending aorta. Dacron graft interposition was performed as surgical treatment. The symptom and signs disappeared dramatically after the operation

Özet

Künt travmalardan sonra oluşabilen gözden kaçmış damar yaralanmaları yaşamı tehdit eden ve acil girişim gerektiren klinik tabloları ortaya çıkarır. Aort transeksiyonu (AT) bu gözden kaçabilecek vasküler yaralanmaların en ölümcül olanıdır. Bu yazıda künt travma sonrası meydana gelen bir AT olgusu sunuldu. Üç yıl önce künt travmaya maruz kaldığını bildiren hasta eforla gelen nefes darlığı ve çarpıntı şikayetiyle başvurdu. Telegrafisinde üst mediastende genişleme, torakal manyetik rezonans anjiyografi ve bilgisayarlı tomografisinde transeksiyon ve inen aortada anevrizmatik genişleme saptandı. Cerrahi tedavide Dakron greft interpozisyonu yapıldı. Ameliyattan sonra hastanın yakınmaları ve bulguları dramatik olarak geriledi. Künt travmalardan sonra gelişebilen AT, yaşamı tehdit eden ve mutlaka araştırılması gereken bir tablodur.
