Abstract

Two different treatment fields and two energies were compared on basis of dose to the target volume and the lens in patients with acute lymphoblastic leukemia, received cranial prophylactic radiotherapy. Simulation films, computerized tomography slides and treatment plans from 8 patients were used retrospectively. A second virtual simulation field was designed for this study in order to compare to the actual simulation films. Treatment plans were arranged with beam energies of 60Co and 6MVx photons for each field individually. The doses of the lens, cribriform plate, hypophysis, optic nerve and meningeal structure were calculated for two different treatment fields and beam energies. The dose to the lens delivered by 60Co was three times higher than the dose by Linak 6MVx, in both treatment fields. In addition, with 60Co, the doses of cribriform plate, hypophysis and maximum meningeal dose were higher than the prescribed dose. There is no significant difference between two treatment fields. In acute lymphoblastic leukemia patients received cranial radiotherapy, Linak 6 MVx should be preferred to obtain both homogeneous dose distribution and maximum protection of the lens.
