Abstract

Purpose: In the case of the skeletal muscle metastasis of lung cancer the authors report radiographic, CT scan and MRI findings of painful masses in the femoral regions. Muscle biopsy revealed extensive infiltration of the muscle with carcinoma cells. Case: A 68 year-old man presented with a large cavitary lesion with thickened wall in the left hilus on radiography and CT scan. The diagnosis of the lung cancer was made by bronchoalveolar lavage. The radiograph did not show any soft tissue mass, except periostea reaction. MRI showed a tumoral mass in the right quadriceps muscle with signal intensity slightly lower on TI -weighted and higher in T2-weighted images than surrounding muscle tissue. Needle biopsy revealed metastatic squamous cell carcinoma. The patient died six month later. Conclusion: MRI is more useful technique than the other methods for determination of the localization, the relationship of the surrounding tissues and the manifestations ofmetastatic masses on the skeletal muscle due to malign tumor of the lung.

Özet

Amaç; Malign tümörlerin iskelet kasına metastazları nadirdir. Akciğerden kaynaklanan malign tümörlü olan olguda klinik bulgular ve iskelet kasındaki metastatik lezyonların klasik radyografık, BT, MRG bulguları irdelendi. Olgu: 68 yaşında erkek olgunun akciğer radyografisinde sol akciğerde, büyük, kalın düzensiz duvarlı kaviter lezyon izlendi. Lezyon bronkoalveolar lavaj (BAL) sonucunda kötü differensiye epidermoid kanser tanısı aldı. Bir yıldan beri bulunan sol uyluk ön yüzündeki yumuşak doku kitlesi nedeniyle yapılan direkt grafıde periost reaksiyonu dışında bulgu saptanmadı. MRG'de kitle lezyonu, çevre kas yapılarına göre Tl Alı kesitlerde hipo, T2A'lı kesitlerde hiperintens izlendi. Yumuşak doku kitlesinden yapılan biopsi sonucu metastatik tümörle uyumluydu. Olgu tanı konduktan 6 ay sonra kaybedildi. Sonuç: Akciğer malign tümörlü olgularda iskelet kasında kitle ve ağrı varlığında metastazdan şüphelenilmelidir. Kitlelerin lokalizasyonunda, çevre dokularla ilişkisi ve kemik tutulumunun saptanmasında MRG ve BT tamamlayıcı tetkiklerdir.
