Abstract

Chronic obstructive pulmonary disease (COPD) is a disease state characterized by the presence of progressive and gene-rally irreversible airflow obstruction due to chronic bronchitis and emphysema. It is the sixth leading cause of death among the world. COPD as a source of immerse morbidity, leads to loss of productive power and enormous treatmentcosts. Along the course of the disease, significant changes develop in diaphragma, especially in emphysematous patients. One of these changes is the destruction of the normal diaphragmatic structure as a result of hyperinflation; and the other one is the changes due to malnutrition that is seen with an increased prevalance among the COPD patients. Depen-ding or these changes in diaphragma, pulmonary rehabilitation programs and nutritional support are also included in the COPD treatment besides various pharmacological agents and oxygen therapy. In this study, it was aimed to evaluate the diaphragmatic contractility with surface electromyography (EMC) in COPD patients. 24 male patients, with a mean age of 65.0 ± 11.62 years who were in stable period of COPD of especially emphysema type, were included in this study. 1 3 male patients without any respiratory system pathology, with a mean age of 63.76 ± 7.75 years were chosen as control group. Arterial blood gas analysis, respiratory function tests and diffucion capacity measurements were performed and also dyspnea scorrings and body mass indices, as a marker of malnutrition, were determined in study group. Surface diaphragmatic EMC measurements were done in both groups. When compared, no statistically significant difference could be detected between the two groups' diaphragmatic EMC values. Similarly, no correlation was found between the diaphragmatic EMC values and the blood gas analysis, respiratory function tests, diffusion capacity tets, body mass indices and dyspnea scorrings of patients in the study group. Patients in the study group were subdivided into two groups according to their body mass indices (BMI < 19.9 and BMI > 19.9). No statistically significant difference was also detected between these two subgroups diaphragmatic EMC values. With these results, it was thought that diaphragmatic contractility shows no alteration electromyographically in emphy-sematous patients and the malnutrition has no effect on the diaphragmatic contractility electromyographically. It should be emphasized that, further controlled studies that will use other methods for the assesment of surface diaphragmatic EMC, are still necessary.
