Abstract

Aim of this study is to assess the serum TNF-$alpha$ levels and the effects of high dose inhaler steroids used in short period, on clinical and inflamatory determinants, in acute exacerbation of chronic obstructive pulmonary disease (COPD) patients. Twenty-six patients, diagnosed as acute exacerbation of COPD with FEV1< 70% and body mass index (BMI) > 19, were included. Conventional therapy in addition to high dose inhaler steroids as 1000 $mu$g/day budesonide were applied to these patients beside conventional therapy. The decrease in white blood cell count and total neutrophil number were found statistically significant. The decrease in serum TNF-$alpha$ levels in posttreatment period was also statistically significant (28.7 ± 7.2; 33.6 ± 1.1 pg/mL respectively). The FEV1 values of patients were improved significantly (pretreatment; 43% ± 22.7; posttreatment 50.2% ± 26.3) (p< 0.05). Before treatment DLCO was 57.1% ± 26.8 of mean predicted value; after treatment it was 67.2% ± 24 (p< 0.01). In blood gas analysis statistically significant improvements were achieved. By using American Thoracic Society (ATS) dyspnea scorring system, pre and post treatment values were found as 3.2 ± 0.9 and 1.8 ± 0.2 respectively (p< 0.001). As a result high serum TNF-$alpha$ levels in patients with acute exacerbation of COPD can be accepted as the determinant of inflammation. Though the effects of high dose inhaler steroids on serum TNF-$alpha$, COPD, inhaler steroids, inflammation and neutrophil levels need to be studied in a controlled manner, our results were consistent with the other studies.
