Abstract

Noninvasive positive pressure ventilation (NIPPV) reduces mortality, intubation rate and length of hospital stay in patients with acute hypercapnic respiratory failure. The guess that success of this procedure is important because prolonged application of NIPPV in unresponded patients may unduly delay the time of intubation and may cause increasing mortality. Our aim was to determine the clinical variables associated with the success or failure of NIPPV in the treatment of acute hypercapnic respiratory failure. Fifty four patients (38 male, 16 female, median age 63.9 ± 9.57 years) who acute hypercapnic respiratory failure evaluated retrospectively. Fourtythree (79.6%) had obstructive and 11 (20.4%) had restrictive respiratory insufficiency. Thirtyfour (62.9%) patients initially responded to NIPPV in terms of objective (arterial blood gas tensions) and subjective improvement. After 5.3 days of NIPPV 6 patients experienced a new episode of acute respiratory failure. Totally, NIPPV was not successful in 20.3% of our patients. There were differences between the success and failure groups for improvements in pCO2-pH after 2 hour of NIPPV (respectively, p< O.OT, p< 6.05) and concomitant pneumonia (p< 0.05). There were no significantly differences between the success and failure groups in age, gender, obstructive restrictive disease and initially blood gas analysis. The overall hospital mortality was 12.9% in our study. We concluded that improvements in pCC>2-pH after 2 hour of NIPPV and accompaning pneumonia were found predictors of the success and failure of NIPPV.
