Abstract

The first step in determining the aetiology of pleural fluids is whether it is transudate or exudate. In this study we explored the value of the acute phase reactants (C-reactive protein and alpha-2 macroglobulin), Light criteria and albumin gradient in differentiation of pleural fluids comparatively. A total of 94 patients introduced to the study who were researched and diagnosed for pleural fluid in Atatürk Chest Disease and Chest Surgery Centre. Seventy-two of 94 were diagnosed as exudate and 22 of 94 were diagnosed as transudate. Pleural fluids were separated into four groups according to their aetiology. Simultaneous blood and pleural fluid samples were taken for C-reactive protein, alpha-2 macroglobulin, lactate dehydrogenase (LDH), albumin, and total protein levels. Consequently the sensitivity and the specificity rates were found as 90.3% and 73% respectively for Light criteria; 70.6% and 90.9% for albumin gradient; 47% and 90.6% for alpha-2 macroglobulin; 68.1% and 99.9% for C-reactive protein. According to these findings, we think that alpha-2 macroglobulin and C-reactive protein levels may be useful in differential diagnosis of exudative fluid rather than differentiation of transudate from exudate pleural effusions.
