Abstract

We investigated cases of significant hyperuricemia during pyrazinamide (PZA) or ethambutol (ETB) therapy of patients with tuberculosis (TB). In this study, 91 patients with newly diagnosed TB in our hospital between February 2004 and January 2005 are prospectively analysed. Patients consisted of 71 males (78%) and 20 females (22%). Of the cases 23 (25%) experienced joint pain during treatment. None had physical findings of redness, warmth, and swelling or decreased range of motion. Baseline uric acid mean levels before treatment was found 4.6 mg/dL, after treatment by day 15th 8.3 mg/dL, by day 30th 8.5 mg/dL and by day 60th 8.3 mg/dL. Following discontinuation of PZA and ETB on day 60, uric acid levels approximately returned to pretreatment levels on day 120th (4.9 mg/dL) and subsequent controls on day 180th, it was found 4.9 mg/dL. The mean begining time of arthralgias was 20.7 day while the resolving time was 77 day. Patients treated with nonsteroidal antiinflammatory and/or uricosuric agents had their symptoms resolved on day 67 and those with no treatment on day 70 and difference was nonsignificant (p> 0.05). Our study demonstrated that hyperuricemia end arthralgias occuring during treatment of tuberculosis with PZA and ETB were not serious enough to necessitate discontinuation of these drugs.
