Abstract

Aim: This study was to determine diagnostic yield of multiple bronchoscopic forceps biopsies in diagnosis of lung cancer presenting as an endobronchial mass. Material and methods: Twenty-five consecutive patients with lung cancer presenting as an endobronchial mass were studied. Six forceps biopsies were performed on each of 25 mass lesions. Each biopsy specimen was placed in a separate coded container. All preparations were examined by the same pathologist. Results: Hundred thirty-seven out of a total of 150 biopsies (91.3%) "were positive for malignancy. The first biopsy specimen obtained positive diagnosis for malignancy in 23 (92%) cases. The second biopsy specimen had additional diagnostic yield of 4%. First three biopsy specimens yield the diagnosis in all cases. The estimated probability of obtaining a diagnosis of malignancy after one biopsy was 0.9133, after two biopsies was 0.9924 and after three was 0.9993. Conclusion: Our results pointed out that three biopsies of lung cancer presenting as an endobronchial mass lesion were adequate to achieve maximum diagnostic yield.

Özet

Amaç: Endobronşiyal kitle ile seyreden akciğer kanserinin tanısında multipl bronkoskopik forseps biyopsi örneklerinin tanı değerinin saptanmasıdır. Gereç ve yöntem: Bronkoskopik incelemede endobronşiyal kitle görülen ardışık 25 hasta çalışıldı. Kitle lezyonlarının her birinden 6'şar forseps biyopsi yapıldı. Biyopsi örnekleri kodlanmış ayrı tüplere alındı. Tüm preparatlar aynı patolog tarafından değerlendirildi. Bulgular: Yüz elli biyopsi örneğinden 137'si (%91-3) nıalignite açısından pozitif idi. Olguların 23'ünde (%92) tanı birinci biyopsi örneği ile elde edildi. İkinci biyopsi %4 ek tanı sağladı. Üç biyopsi yapıldığında tanı değeri %100 olarak bulundu. Malignite açısından pozitif tanı elde etme olasılığı bir biyopsi sonrası 0.9133, iki biyopsi sonrası 0.9924 ve üç biyopsi sonrası 0.9993 idi. Sonuç: Çalışmamız, endobronşiyal kitle ile seyreden akciğer kanserinin tanısında üçten fazla bronkoskopik biyopsi yapılmasına gerek olmadığını göstermektedir.
