Abstract

There are many kinds of tracheoesophageal fistula (TEF). They are ususally diagnosed during childhood, because most are associated with esophageal atresia. H type fistula in which the membranous part of the trachea is connected to the esophagus with a thin fistula may not be diagnosed til adolescent or adulthood because it is not associated with esophageal atresia. These patients usually presnt with retrosternal pain, recurrent pneumonia and recurrent bouts of coughing after drinking. We present a male patient without any symptoms until adulthood with a H type TEF. The patient who presented with fever and left lateral chest pain, had increased bronchial markings on left diaphragm in chest X-ray. Bronchoscopic examination was performed because TEF was observed with thorax computerized tomography. In bronchoscopy the orifice of TEF that closed with coughing reflex at the middle part and the posterior wall of trachea was visiualized. The esophageal opening of the fistula that was located 15 cm from the tooth was seen at the proximal part esophagus in the esophagoscopy. Tef is usually treated surgically during childhood. The most common post-operative complication in these patients is tracheomalacia. There may be symptoms such as disphagia, esophagitis and gastroesophageal reflux postoperatively, because of dismotility of the esophagus. These problems may continue till adulthood and recurrent aspirations can occur in the lungs, because of residual dysfunction of the esophagus.
