Abstract

Pulmonary sequestratıon is defined as an area of abnormal pulmonary tissue not connected with bronchial system, supplied by an aberrant systemic artery and without a normal pulmonary function. It is classified as extralobar and intralobar. Sequestrations are most frequently asymptomatic and discovered incidentally. While hemoptysis may be common presenting symptom, massive hemoptysis is rarely seen. We present a case of 21-years-old previously asymptomatic man whose initial presentation were massive hemoptysis and right chest pain. Chest x-ray revealed homogenous opacity in lateral segment of right lower lobe. Bacteriological and biochemical tests were normal. The thorax computed tomography revealed a mass lesion in the lateral segment of right lower lobe. Bronchoscopically, endobronchial system was normal. Color flow Doppler signals were observed in the mass lesion by color Doppler ultrasonographic evaluation. These findings suggested a diagnosis of pulmonary sequestration. The aortagram showed an abnormal artery originating from the descending aorta, and entering the right lower pulmonary lobe. By selective angiography, the anomalous artery was determined. The diagnosis was confirmed, and the patient was referred for operation. Pulmonary sequestration should be considered in the differential diagnosis of mass lesion or massive hemoptysis.
