Abstract

Aim: The aim of this study was to demonstrate the experience of our clinic in interventional bronchoscopy and to evaluate the typical carcinoid and benign endobronchial tumour patients treated with these techniques.Materials and Methods: The files of typical carcinoid or benign endobronchial tumour cases treated with interventional bronchoscopic techniques in our clinic between the years 2008-2012 were investigated retrospectively. Demographic characteristics, bronchoscopic procedure, location and type of the lesion, pathological diagnosis, treatment modality, the success of the interventional bronchoscopic treatment and follow-up periods of the patients were recorded and evaluated. Results: Eleven (64.7%) male and six (35.3%) female patients, for a total of 17 cases with a mean age of 57.1±11.5 (min: 34, max: 76) years were included in the study. The process was repeated three times in three cases, twice in two cases and six times in one case. Twenty-one (70.0%) of the interventional procedures were performed with flexible bronchoscope, while nine (30.0%) were performed with a rigid bronchoscope. Nine of the bronchoscopic procedures were performed under general anaesthesia, whereas 21 were performed under local anaesthesia. The lesions were located in the right bronchial system in ten (58.8%) cases, in the left bronchial system in six (35.3%) cases and in the trachea in one (5.9%) case. Of the lesions, nine (52.9%) were of the polypoid type, five (29.4%) were a vegetative mass and three (17.7%) were located in the bronchus with a broad base. A total of 33 processes, of which 15 were argon plasma coagulation, 14 were electrocautery and four were cryocautery, were applied to 17 patients. Six lesions were diagnosed as typical carcinoid tumours, five as lipomas, four as hematomas and two as papillomas upon pathological examination. There were no major complications seen after the interventional bronchoscopic procedures. While interventional bronchoscopic techniques were successful in 16 (94.1%) patients, one (5.9%) patient underwent surgical intervention. Conclusion: Interventional bronchoscopic procedures are effective methods with a high success rate which do not result in a loss of pulmonary function and have low morbidity and mortality. In the treatment of typical carcinoid and benign endobronchial tumours, interventional bronchoscopic methods should be considered as the first choice of treatment modality

Bronkoskopik işlemlerin 9'u genel anestezi altında yapılırken, 21'i lokal anestezi altında yapılmıştı. On (%58.8) olguda lezyon sağda iken 6 (%35.3) olguda solda, 1 (%5.9) olguda ise trakeada yer almaktaydı. Patolojik inceleme sonucunda lezyonların 6'sı tipik karsinoid tümör, 4'ü hamartom, 5'i lipom ve 2'si papillom tanısı almıştı. Girişimsel bronkoskopi işlemleri sonucunda major komplikasyon görülmemişti. Girişimsel bronkoskopik yöntemler olguların 16'sında (%94.1) başarılı olurken 1 (%5.9) olguya cerrahi uygulanmıştı.Sonuç: Girişimsel bronkoskopik işlemler mortalite ve morbiditesi düşük, solunum fonksiyon kaybına neden olmayan, başarı oranı yüksek, efektif yöntemlerdir. Tipik karsinoid tümör ve benign endobronşiyal tümörlerde ilk tercih edilecek tedavi modalitesi olarak akılda tutulmalıdır
