Abstract

Aim: It was reported that serum leptin increases during exacerbations of chronic obstructive pulmonary disease (COPD) by various mechanisms. In the study, the aim was to assess the earlier effect of non-invasive positive pressure mechanical ventilation (NIMV) on serum leptin and other blood tests in patients with acute hypercapnic respiratory failure related to COPD. Material and methods: Of the patients who were being treated for exacerbation of COPD and respiratory failure at Intensive Care Unit, the ones having NIMV were included in the study. Serum leptin, arterial blood gases and biochemical parameters were measured before and after the treatment. Results: A total of 48 patients 39 male (81.3%) and 9 female (18.8%) with a mean age of 64.8 years were included in the study. As the parameters of arterial blood gases getting better, the mean serum leptin was 9-332±17.9 ng/mL before initiation of NIMV, was 9-037±18.4 ng/mL at the 4th hour of NIMV and was 8.907±14.9 ng/mL at the 72nd hour of NIMV (p=0.600). Glucose, alanine amino-transferase, aspartat amino and C-reactive protein were significantly lower at the 72nd hour of NIMV. There was no difference for other serum parameters. Conclusion: Serum leptin level increases, independent of the body mass index, in COPD patients with hypercapnic respiratory failure. Serum leptin does not change in the earlier period of NIMV treatment.

Özet

Amaç: Kronik obstrüktif akciğer hastalığı (KOAH) alevlenmesi sırasında plazma leptin konsantrasyonunun çeşitli nedenlerle yükseldiği bildirilmiştir. Çalışmada, KOAH'a bağlı akut hiperkapnik solunum yetmezliğinde, non-invazif pozitif basınçlı mekanik ventilasyon (NİMV) tedavisinin, erken dönemde serum leptin ve diğer kan testleri üzerindeki etkisinin incelenmesi amaçlandı.
