Abstract

Aim: Aim of the study is to determine the effectiveness and differences on the effectiveness, survival, toxicity and quality of life assessment of vinorelbine plus cisplatine (VP) and gemsitabine plus cisplatine (GP) chemotherapy regimens in local advanced NSCLC. Material and methods: Twenty four patients received vinorelbine 30 mg/m2 (day 1 and 8) plus cisplatine 80 mg/m2 in VP arm and gemsitabine 1250 mg/m2 (day 1 and 8) plus cisplatine 80 mg/m2 in GP arm. Cycles were recevied in every 21 days up to four cycles. Quality of life assessment (EQRTC-QLQ C30 and EORTC-QLQ LC) was performed before each cycle. Patients were followed-up two years after therapy and results were evaluated retrospectively. Results: In VP arm partial response was found in ten patients (45.5%), stable disease in eight patients (37.5%) and progressive disease in four patients. In GP arm partial response was found in 11 patients (45.8%), stable disease in nine patients (37.5%), progressive disease in four patients. Median survival time in each arm was 9 and 11 months, time to progression 6 and 5 months, one year survival was 41.6% and 30.9% respectively. There were no differences between groups in response rates, median survival time and one year survival time. Grade I anemia was the most common hematological toxicity in each group (13%-20.71%). There were significant differences in grade 3-4 neutropenia between arms which was significantly higher in VP arm (16.6%-3.6%). There were statistical differences in nonhematological toxicities; constipation was higher in VP arm, fever was higher in GP arm. Quality of life assessment found deterioration in social function in VP arm, improvement in other functions. In GP arm there was improvement in functional status with chemotherapy. Conclusion: There were significant differences in neutropenia in VP arm but percentages were consistent with literature. No differences were identified in response rate, survival, quality of life scale between VP and GP regimens in local advanced NSCLC.

Bulgular: VP kolundaki on hastada kısmi yanıt (%45.5), sekiz hastada stabil yanıt (%36.4), dört hastada progresif yanıt; GP kolunda 11 hastada kısmi yanıt (%45.8), dokuz hastada stabil yanıt (%37.5), dört hastada progresif hastalık saptandı.
