Abstract

Objective: Pulmonary thromboembolism (PTE) is a disease with high morbidity and mortality. Mortality is 30% in untreated PTE. The aim of this study is to evaluate retrospectively the PTE patients treated in our clinic for symptoms, risk factors, physical examination, laboratory and radiologic findings. Methods: The files of PTE patients, diagnosed in the Pulmonary Diseases Department of an Education and Research Hospital between 2001-2006, were evaluated retrospectively. SPSS 10.0 program and Mann-Whitney U Test were used for statistical analysis. Results: Seventy-eight patients, 41 female 37 male, with a mean age of 53.4±15.3 were included in the study. The most common risk factors were immobilisation (34.6%), presence of deep venous thrombosis (30.7%), age >65 years (28.2%) and the history of surgical operation (24.3%). The common symptoms were chest pain (82.1%), dyspnea (67.9%), tachycardia (35.9%), cough and hemoptysis (24.4%). Tachycardia, end-inspiratory ralles, deep venous thrombosis (DVT) findings and tachypnea were the most common physical examination findings. In 23 patients (29.4%), pleural efusion was detected. D-dimer level was elevated in 51 (98.1%) of 52 patients. Echocardiography was performed in 43 (55.1%) patients. The most common finding was pulmonary hypertension (95.3%). Bilateral lower extremity venous doppler sonography was performed in 60 (76.9%) patients and unilateral thrombus was detected in 40% of them. Of the patients, 51.3% were diagnosed by ventilation/perfusison (V/Q) scintigraphy whereas 62.8% were diagnosed by spiral computed tomography (CT) 63.6% of patients had hypoxemia, 54.5% had hypocapnia and 4.5% had hypercapnia in arterial blood gass analyses. The most common chest x-ray findings were pleural effusion (29.5%) and consolidation (19.2%). Most frequently, sinus tachycardia was detected as an electrocardiography (ECG) finding in our patients. Conclusion: All the findings of our study were similar to the literature. Morbidity and mortality can be reduced, when the possibility of PTE is kept in mind as a diagnosis, especially in patients with risk factors
