Abstract

Aim: Continuous ambulatory peritoneal dialysis (CAPD) is commonly used for the maintenance of patients with chronic renal failure. Common causes of catheter dysfunction are obstruction due to catheter migration to an extrapelvic site or blockage of the catheter drainage. Herein, we report a review of our experience with the use of mechanical methods in the treatment of CAPD patients with peritoneal dialysis catheter obstruction. Methods: Eight patients (median age 55 years) with peritoneal catheter dysfunction were included in the study. 0.24 cm x 40 cm semi-flexible steel wire and 0.1 cm x 40 cm wire guide was inserted through the obstructed catheter under aseptic conditions. Results: The catheter drainage of four out of 5 patients with obstruction due to catheter displacement was corrected entirely, whereas, the drainage of the remaining patient with displacement could be corrected partially by placing the catheter closer to its original location. Two patients' catheters blocked with fibrin clots and 1 patient's catheter blocked by a kink were also corrected successfully. No complication was encountered. Conclusion: Mechanical methods, for re-functioning of the obstructed catheter in CAPD patients seem to be reliable, safe and cheap. Larger series of studies are needed for more accurate results.
