Abstract

Objectives: To determine the correlation between endoscopic and histopathological examinations in establishing the diagnosis, level of activity, and spread of lesions and to determine parameters which demonstrate acute and chronic periods of ulcerative colitis (UC). Study Design: Of four-hundred endoscopic biopsy specimens from nonneoplastic lesions, 112 cases were endoscopically diagnosed as UC. The diagnosis was confirmed histopathologically in 75 cases. Endoscopic and histopathologic parameters showing acute and chronic features and reactivation were evaluated. Results: The sensitivity, specificity, positive predictive value, negative predictive value, accuracy, and kappa value for endoscopic examination were 88%, 88%, 84%. 62%, 76%, and 0.47, respectively. Lesion spread was correlated in 56%, whereas in 33%, there was no correlation (kappa 0.42). Histopathological parameters most commonly encountered were distortion and atrophy of the crypts, loss of goblet cells, and increased lymphoplasmacytic infiltration in the lamina propria. In the differentiation of acute and chronic periods, ulcer, cryptitis, crypt abscess, Paneth cell metaplasia and increased polymorphonuclear neutrophils and eosinophils in the lamina propria were found to be statistically significant features. Conclusion: It is of great importance to incorporate clinical, laboratory, endoscopic and histopathological findings into the diagnosis of UC.

Özet

Amaç: Ülseratif kolit (ÜK) tanılı olgularda, endoskopik ve histopatolojik incelemenin tanı, aktivite derecesi ve lezyonların yayılımını saptamadaki uyumunu belirlemek ve ÜK'nin akut ve kronik dönemlerini gösteren histopatolojik parametreleri saptamak. Çalışma Planı: Nonneoplastik öntanılı 400 kolon endoskopik biyopsi materyali retrospektif olarak incelendi. Endoskopik ÜK öntanılı 112 olgunun histopatolojik tanı alan 75'i çalışmaya alındı.
