Abstract

Objectives: This study was designed to evaluate whether disturbances in the sensory distribution of the median nerve affect two-point discrimination (TPD) in the sensory area of this nerve in carpal tunnel syndrome (CTS). Patients and Methods: The study included 26 patients (12 women, 14 men; mean age 52 years; range 36 to 77 years) in whom CTS was confirmed by nerve conduction studies and needle electromyography. Correlations were sought between the median sensory nerve action potential parameters (peak latencies and negative peak amplitudes) from the second and third digits and TPD findings. Results: Of 20 patients with bilateral CTS, median sen-sory nerve action potentials could not be elicited in one hand in three patients, and in both hands in two patients. Two-point discrimination showed deterioration in these seven hands. There was a statistically significant correlation between the peak latencies of the sensory nerve action potentials of the second and third digits and the TPD findings. However, TPD results did not correlate with negative peak amplitudes. Conclusion: Although the deterioration of two-point discrimination within the sensorial distribution of the median nerve innervation can be detected in severe cases, TPD studies may not be sufficient alone to make a diagnosis of CTS.
