Abstract

Purpose: To compare the visual results and complication rates of combined and two-step procedures in patients with coexisting cataract and vitreoretinal pathology. Materials and Methods: The results of combined phacoemulsification and pars plana vitrectomy in 14 eyes of 12 patients (Group 1) and two-step procedures in 12 eyes of 11 patients (Group 2) were prospectively compared. Vitreoretinal surgery was performed a mean of 35 days after phacoemul sification. Pre-and postoperative visual acuities, complications encountered, intraocular pressures (IOP) and follow-up time of the patients were all recorded. Results: The most common indication for vitreoretinal surgery was diabetic vitreous hemorrhage and tractional retinal detachment in both groups. Postoperatively, visual acuity improved in 9 eyes, unchanged in 3 eyes and decreased in 2 eyes in group 1. In group 2, visual acuity improved in 8 eyes, decreased in one eye and unchanged in 3 eyes. The most common postoperative complication was transient intraocular pressure (IOP) elevation in group 1 and group 2. Postoperatively, the rates of IOP elevation, intraocular lens malposition and posterior synechia were significantly higher in group 1 (P<0.05). Conclusion: Phacoemulsification combined with pars plana vitrectomy is a preferable surgical procedure with good visual results in patients with coexisting cataract and retinal pathology. However, the possibility of higher complication rates should not be ignored.
