Abstract

Purpose: To investigate the results of intraoperative mitomycin-C (MMC) use in viscocanalostomy. Material and Method: Retrospective results of 16 eyes with trabeculectomy (Group 1), and 17 eyes with viscocanalostomy (Group 2) were compared with the prospective results of 15 patients who underwent viscocanalostomy with adjunctive MMC (0.2 mg/ml) (Group 3) which was applied over and under the outer sclera flap for three minutes intraoperatively. Intraocular pressure (IOP) equal to or lower than 18 mmHg was accepted as surgical success. Results: Mean preoperative IOPs which were respectively 37.5, 36.3 and 39.1 mmHg in the three groups (p=0.609) significantly decreased in each group to the levels of 9.7, 11.9 and 11.6 (p=0.288), with 70, 62 and 69 percent decrease ratios (p=0.376) after mean follow up periods of 19.4, 19.0 and 8.5 months respectively. Surgical success rates without medications were 62.5% in Group 1, 59% in Group 2, and 80% in Group 3 (p=0.506). The predominant conjunctival bleb types presented as diffuse blebs in the trabeculectomy group, low-lying blebs in the standard viscocanalostomy group, and as typical avascular blebs in the viscocanalostomy group with MMC. Cataract progression appeared to be more frequent in the trabeculectomy group, however, no significant difference was found in terms of complications. Conclusion: Intraoperative MMC use in viscocanalostomy might increase the efficacy and widen the indication range of the technique, however, prospective studies with longer follow-up are required
